Dear Parents/Guardians:

Welcome to the 2009-10 school year!! Our staff is excited to get underway to meet the learning needs of your child. In order to have a successful year we need your help in providing some critical information about your child.

We are asking that all parents/guardians fill out their child's emergency information forms on-line. We will be using this information to update our records. Please use the information on the attached label to complete the survey. If you do not have access to the internet, please send a note to your child's teacher requesting a hard copy. Please follow the instructions below.

  1. Log onto EBPS.net
  2. On the right side of the screen there is a box that says “Site Shortcuts” In this section please double click on “Mitchell Middle School.”
  3. On the left side of the screen there is a box which says “Announcements.” In this section please double click “Parent Information Form.”
  4. Complete the survey. All fields with an * need to be completed. Press “Submit.” (One of the fields requires you to acknowledge receiving our Student Handbook which we have included within the Student Agenda which all students received today.)
  5. Repeat steps 2-4 for all other children in the Mitchell Middle School.

One of the optional fields we have asked you to fill-in is your email address. In order to improve our ability to communicate with one another we are going to more fully enter the 21st Century. Our hope is to increase the level of electronic communication between school and home. We will use email to send notices and reminders to you so that we can be certain that you receive them in a timely manner and save time and money. I assure you that your email address will remain confidential.

The only other form that the school district has sent home is an application for “Free and Reduced Lunch.” We are aware that many families are struggling at this time so please take advantage of this program if you feel you are eligible.

Please complete the on-line information by September 7th so that we can be certain to have critical information about your child immediately available to us. If any of your child's information changes during the school year please notify our office by telephone.
